* If the amount to be sacrificed is a fluctuating amount, e.g. a bonus show �x% of bonus to be declared' OR �£x up to a maximum of £y'.
Where the salary sacrifice is £5,000 or more (this limit is reviewed by the Inland Revenue from time to time) or if the salary sacrifice is in respect of fluctuating earnings e.g. bonus, director's fees the Employer/Employee should keep a copy of the completed letter and send the original to the Inland Revenue (appropriate Schedule E District, Local Inspector of Taxes) for agreement.
